How To Get Started:
To take advantage of our free services,
you must be registered with the Foreign Medical Plan (FMP)
Below Are The Steps:
1. Fill out the Foreign Medical Program form, VA Claim File Number is not required if your SS# is provided.
2. Make sure you sign and date the document.
3. Email the completed form to hac.fmp@va.gov.
4. It can take up to 4-6 weeks to be fully registered in the FMP system.
5. Once accepted you will receive a confirmation email from the FMP program, once received be sure to bring it to your first visit.
